Bupivacaine HCl 0.5%, dextrose 8%
Indications/Uses
Local or regional anesth or analgesia for surgical procedures; for diagnostic & therapeutic; for obstet procedures.
Dosage/Direction for Use
Peripheral nerve block (0.25-0.5% soln) 5-20 mL for 12.5-50 mg in 180-360 min. Caudal block (0.25-0.5% soln) 15-30 mL, max of 225 mg in 360-720 min. Epidural block (0.25-0.75% soln) 15-30 mL for 37.5-225 mg in 180-300 min. Spinal block (0.5% soln) 3-4 mL for 15-20 mg in 75-150 min; (0.75% soln) 2-3 mL for 15-22.5 mg in 150 min.
Contraindications
Obstet paracervical block anesth. Sensitivity to amide-type anesth. Not recommended for IV regional anesth. Childn <12 yr. Elderly, acutely ill patients.
Special Precautions
Excessive dose during pregnancy can result in high fetal blood conc & fetal depression.
Adverse Reactions
Restlessness, dizziness, tinnitus, twitching or tremors & progress to convulsions, excitement (transient or absent) w/ depression or drowsiness, unconsciousness, resp arrest, confusion, numbness & miosis. Decreased cardiac output & hypotension, sinus bradycardia, arrhythmia or asystole/cardiac arrest, tachycardia & early signs of HTN. Skin/muscle irritation, anaphylaxis, nausea & vomiting, blurring of vision.
Drug Interactions
May enhanced sympathomimetic effects w/ β-adrenergic blockers. May lessen effects w/ chloroprocaine. Can cause severe sustained HTN w/ cyclic antidepressants & MAOIs. Can cause arrhythmias w/ enflurane, halothane, isoflurane & related drugs.
